DUI Enrollment Process

1. Start Enrollment: For non-referred or non-court-ordered DUI client, please click here to begin the enrollment process. If you are court-ordered, or have a supervising officer, please request they submit a referral through our website. 
2. Paperwork: Use the link above to fill out and submit the required enrollment paperwork. 
3. Confirm Payment: Self-paying client must submit a $75 deposit towards your assessment or provide verification of insurance to confirm your payment method. 
4. Scheduling: After completing your paperwork and confirming payment, our office will contact you to schedule your assessment. 
5. Begin Treatment: Following your assessment, you will be placed into a treatment program to start services. ​
